Hospital-Acquired Conditions Expenditures Tool

How can your facility avoid penalties?

This tool is for facilities whose hospital-acquired condition (HAC) rates resulted in a financial penalty in the previous year via the Centers for Medicare & Medicaid Services (CMS) HAC Reduction Program. This report incorporates the facility’s preview or final HAC Reduction Program report data to provide an indication of whether user-proposed reductions in the facility’s standardized infection ratios (SIRs) would have been sufficient to have avoided the financial penalty.

Instructions:

  • Enter data from Table 1 (Performance on Total HAC Score) and Table 2 (Hospital Measure Results and Winsorized z-scores) of your facility’s Hospital-Specific Report (HSR) for the Hospital-Acquired Condition Reduction Program, and
  • Provide proposed a target SIR or target percent reduction for each HAI type applicable to your facility.

Instructions:

  • The SIR is referred to as the Measure Result in Table 2 of your HSR (e.g., 5th percentile Measure Result).
  • The Winsorized z-score will automatically be calculated by this tool using the data you enter. It is recommended you double-check that value against the Winsorized z-score in Table 2 of your HSR to verify you have entered the data correctly.
  • If a Measure is not tracked by your facility, leave data input in that row blank.

Based on this data, your facility would have incurred a HAC Penalty.

If these proposed reductions had been in place, your facility would have incurred a HAC Penalty
